Download Rx form Fill out the Rx Form

Step TWO

Follow the link to the google form
Upload the completed Rx Form
Upload other files, such as photo, video, etc.

If you scanned with on of the scanners listed below, you can skip uploading your jaw scans to the Google form.
Send the scans directly from your scanner to our lab. Use the instructions.

If you have any items that you visit, send them to the courier to the laboratory, not forgetting about point THREE.
