Download Rx form Fill out the Rx Form. And don't forget to sign it. According to Florida Administrative Code: R. 64B5-17.006.
Step TWO
All materials for the preparation of the work such as: photographs, CT x-rays and other files can be compressed and sent to us as an archive to our email ordersfalconlab@gmail.com. Please write in the subject line: Order_Clinic name_Patient name.
